I hesitate to start this thread but I think that we all understand the learning there is to be had from accident reports and I want to circulate a list of reports around our flight test team at my company (Hybrid Air vehicles). There was an email from the society today saying that there have been seven fatal flight test accidents in the last seven years. I could recall four of them (Virgin Galactic, AW609, G650 and A400M). I found another one by internet searching, the Bell 525, but that leaves two which I cannot find. There was an Icon lost with a test pilot flying but the news reports suggest that was not a test flight.
What have I missed, please?

Joe - I believe that message came from the Flight Test Safety Committee to attract attendance to the upcoming flight test safety workshop. I don't know more than the accidents you already mentioned so assume the others were on small projects somewhere. On this vert same topic, the SFTE Technical Council (which I Chair) is trying to accumulate case studies of FT mishaps and close calls to identify recurring red flags to head of future problems. Al, I believe Ben Luther and Tom Huff keep detailed records for the ASO at Gulfstream - they may be a good resource, also Andre Tousignant at Bombardier. I'd also be happy to help contribute.

Referring to the SFTE Technical Council collecting case studies, the public AAIB report for our (non-injuring) flight test accident is at the link below:

I was not on that flight but I was log keeper in the control room overlooking the airfield. It was very hard to stay at my duty and keep scribing while everyone else ran to assist.
